Requirements
- WI license
- 1 year experience
- Home Health experience
- Training working with the adult/geriatric population with a variety of orthopedic, neurological and functional mobility diagnosis
- Knowledge of Physical Therapy standards and practices acquired through a minimum of a Masters degree in Physical Therapy
- Master's Degree in Physical Therapy
- AHA BLS
- Float
- Charting/Equipment: Laptop for point of service documentation, NetSmart EMR, and cell phone for GPS and communication
- Unit Specific Skillset/Unit Overview: Evaluates, treats and plans patient care in accordance with Physical Therapy Standards of practice. Population served includes adult and geriatric patients. Documents to meet regulatory and reimbursement requirements. Meets productivity expectations of an average of 4.5 visits per day
