Position Summary: The Wound Care Advanced Practice Provider works as a member of the medical team to provide direct and indirect patient care that includes assessment, diagnosis, education, planning and treatment of wounds, skin and ostomies. Serves as an expert in the field of wounds and provides consultative services to GRHC medical staff. Ensures appropriate documentation of services and treatments provided to patients in order to support organizational operations and the continuation of care.
Critical Tasks:
- Evaluates patients, orders and interprets tests, and provides diagnoses of wound and skin conditions.
- Develops and manages an individualized wound care treatment plan for optimal healing outcomes or chronic maintenance.
- Collaborates closely with GRHC medical staff providers, wound nurse, and care teams on wound management practices.
- Maintains knowledge of current wound and ostomy care to provide appropriate therapies, including procedures, durable medical equipment, and medications to assist in healing of wounds.
- Provides hands on wound care to patients, including debridement of wounds by enzymatic, sharp, or autolytic methods when appropriate.
- Orders, reviews, interprets, and integrates diagnostic studies and laboratory findings for initiating or modifying the treatment plan.
- Collaborates with the patient’s primary care provider when comorbidities are contributing to or affecting the healing of a wound to determine a comprehensive treatment plan.
- Provides the required billing information for services provided by the Wound Care Provider and maintains the documentation necessary for those services billed.
- Reviews patient’s prior medical records and maintains a current medical record related to the wound diagnosis and contributing comorbidities.
- Assists with the identification of wound care quality measures and provides data and documentation for tracking quality measures.
- Works with organizational leadership to determine opportunities for education of GRHC medical staff providers, nurses, and clinical support staff with appropriate identification of wound types, prevention of wounds, and appropriate wound care.
- Instructs patient and family regarding wound treatments and management plan for optimal wound outcomes.
- Participates in professional development activities and maintains professional affiliations.
- Completes required number of continuing medical education (CME) hours for license and certification.
- Collaborates with GRHC medical staff providers, clinical support staff, and specialty personnel to obtain consultative services for cases that require a multidisciplinary approach.
- Participates in the development, review, and evaluation of program policies and procedures to ensure the effective implementation of program plans to achieve organizational goals.
- Coordinates with GRHC medical staff providers and other clinical staff to deliver effective treatment.
- Provides patients and their families with relevant information regarding community and state resources, programs, and services.
- Performs other job-related activities as requested.
Required Qualifications:
- Master of Science in Physician Assistant Studies as a certified PA, or Nursing (MSN) as a Family Nurse Practitioner (NP).
- Three (3) years of experience in wound care management preferred.
- Current, unrestricted and valid State of Arizona license to practice as a Physician Assistant from the Arizona Regulatory Board of Physician Assistants or as a Nurse Practitioner from the Arizona State Board of Nursing. If licensed in other states, must possess such Arizona license within one (1) year of hire.
- Current certification by the National Commission on Certification of Physician Assistant (NCCPA) or as a Nurse Practitioner by a recognized certification board.
- Advanced Wound Care certification with the National Alliance of Wound Care and Ostomy (NAWCO) or with the American Board of Wound Management (ABWM) is to be currently active or to be obtained within one year of hire.
- Current or eligible for Drug Enforcement Administration (DEA) licensure or ability to obtain upon hire.
- Current BLS (CPR) certification or ability to obtain within 30 days of hire.
- Demonstrates ability to relate to diverse cultures and specifically the Gila River Community and/or other Native American cultures.
