Clinical Case Manager-Residential Program (Adult) 14CM3Gilead Community Services is seeking an attentive and recovery-oriented Clinical Case Manager to join our multi-disciplinary team in our supervised apartment program for women. About the position : The Clinical Case Manager for Gilead's Middletown area adult supervised apartment program works Sun - Thurs 3p-11p (40 hours) providing case management, advocacy, educational, and supportive services in a program serving women with major mental illness, co-occurring substance use disorders and histories of significant trauma. The individual in this role carries out all phases of clinical services including intake, engagement, assessment, treatment planning, case management, referral, and discharge.
