About the position : The Clinical Case Manager for Gilead's Middletown adult supervised apartment program works Sunday - Thursday 7a-3p providing case management, advocacy, educational, and supportive services in a program serving men with major mental illness who require intensive supports to reside in a community setting. The individual in this role carries out all phases of clinical services including intake, engagement, assessment, treatment planning, case management, referral, and discharge.
