About the position:
The Community Navigator for Gilead’s Middletown area mental health community support service works 40 hours: Monday – Friday 8AM-4PM with some flexibility required providing case management, advocacy, educational, and supportive services to adults with major mental illness and co-occurring substance use disorders. The individual in this role carries out all phases of clinical services including intake, engagement, assessment, treatment planning, case management, referral, and discharge. Services are provided in home and in the community as well as in office. This role also requires the timely completion of required documentation including assessments and other treatment planning.

Minimum Requirements:
- Bachelor’s degree in a human service field; Master’s degree preferred
- One year of clinical experience
- Must maintain and provide proof of valid driver’s license and automobile insurance in good standing
- Reliable use of personal vehicle as needed
- Experience with co-occurring psychiatric and substance abuse disorders required
- Ability to work flexible hours when necessary

Responsibilities:
- Providing clinical knowledge regarding severe and prolonged mental health and substance abuse disorders through training and education
- Providing recovery-oriented clinical case-management services with an emphasis on dignity, respect, and inclusion of family and other natural supports to a case load of 25 or fewer clients
- Providing resource navigation, support and advocacy for individuals’ needs in areas including entitlements, housing, food, clothing, pursuit of personal interests, vocational interests and finance management
- Coordinating the clinical care for clients’ mental health needs and assessing for risks, implementing risk management plans as necessary
- Monitoring positive outcome measures and ensuring program compliance through accurate and timely data reporting
- Completing assessments, treatment plans, and other documentation electronically and on paper in a timely manner
- Acting with compassion, integrity, and ethics
