What are the responsibilities and job description for the Case Manager position at Girls Inc of Sarasota?
The Case Manager will work closely with Program Managers, VP of Programs, contracted mental health therapists, and families to coordinate and oversee the Family Strengthening Program, which offers a continuum of services designed to address the social-emotional health of girls aged 5-14 years old, while strengthening each family’s communication through awareness, parenting tools, behavioral interventions, and mental health referrals to community resources. As well as conducting and planning a monthly parent night for families.
Primary Responsibilities:
- Coordinating and providing care that is safe, timely, effective, efficient, equitable, and client-centered
- Handling case assignments, draft service and behavior plans, review case progress and determine case closure
- Helping clients achieve wellness and autonomy through individualized and group settings and activities
- Implementing developmentally appropriate social-emotional curriculum to meet needs of clients
- Triage girls to on site mental health therapists as needed
- Communicate with schools across the county to ensure continuum of care
- Communicate progress and updates with parents
- Organize monthly parent nights in coordination with the K-8 Program Manager
- Identify and refer girls or families to external resources as needed
- Track data for grant reporting purposes on a monthly basis
Requirements:
- Bachelor’s Degree in education, psychology, counseling or social work
- Experience working with youth in a school or structured setting
- Previous experience with psychological aspects of care
- Effective communication skills
- Excellent organizational and time management skills
- Familiarity with professional and technical emerging knowledge
- Problem solving skills and ability to multi-task
- Compassionate with teamwork skills
- Aware of the trauma-informed approach and resiliency-based solutions
