The Case Manager will work closely with Program Managers, VP of Programs, contracted mental health therapists, and families to coordinate and oversee the Family Strengthening Program, which offers a continuum of services designed to address the social-emotional health of girls aged 5-14 years old, while strengthening each family’s communication through awareness, parenting tools, behavioral interventions, and mental health referrals to community resources. As well as conducting and planning a monthly parent night for families.

Primary Responsibilities:

  • Coordinating and providing care that is safe, timely, effective, efficient, equitable, and client-centered
  • Handling case assignments, draft service and behavior plans, review case progress and determine case closure
  • Helping clients achieve wellness and autonomy through individualized and group settings and activities
  • Implementing developmentally appropriate social-emotional curriculum to meet needs of clients
  • Triage girls to on site mental health therapists as needed
  • Communicate with schools across the county to ensure continuum of care
  • Communicate progress and updates with parents
  • Organize monthly parent nights in coordination with the K-8 Program Manager
  • Identify and refer girls or families to external resources as needed
  • Track data for grant reporting purposes on a monthly basis

Requirements:

  • Bachelor’s Degree in education, psychology, counseling or social work
  • Experience working with youth in a school or structured setting
  • Previous experience with psychological aspects of care
  • Effective communication skills
  • Excellent organizational and time management skills
  • Familiarity with professional and technical emerging knowledge
  • Problem solving skills and ability to multi-task
  • Compassionate with teamwork skills
  • Aware of the trauma-informed approach and resiliency-based solutions
