Job Description :
We are looking for a dedicated Physical Therapist to serve patients in the Rockland area. The role requires a minimum commitment of 15 visits per week. This per diem position involves providing high-quality care to patients in their homes, helping them achieve their physical therapy goals.
Key Responsibilities :
- Conduct thorough evaluations of patients' physical conditions and develop individualized treatment plans.
- Implement therapy interventions aimed at improving mobility, strength, and overall function.
- Collaborate with interdisciplinary teams to provide holistic care.
- Educate patients and caregivers on exercises, home safety, and self-care strategies.
- Maintain detailed and accurate patient records as per documentation requirements.
