Job Summary
We are seeking a dedicated Home Health RN to provide comprehensive patient care and coordinate home health services. This role involves assessing patient needs, implementing care plans, and educating patients and caregivers to ensure the best possible outcomes. Reporting to the Clinical Manager, you will play a crucial role in delivering high-quality, patient-centered care.
Responsibilities
- Conduct home care visits, assess patient progress, and adjust care plans as needed.
- Implement and manage physician-ordered care plans.
- Coordinate and supervise Home Health Aides (HHAs) in patient care and discharge planning.
- Observe, assess, and document patient conditions, notifying physicians of any significant changes.
- Educate patients and caregivers on disease processes, medications, and home care treatments.
- Ensure compliance with HIPAA regulations and medical documentation standards.
Qualifications
- Current RN license in good standing.
- Strong knowledge of managed care, patient assessment, and discharge planning.
- Experience in hospital settings, critical care, or post-acute care is preferred.
- Proficiency in medical terminology and HIPAA regulations.
