Mon-Fri 8am-5pm
Hourly rate depends on experience, range is $23-25/Hour

POSITION SUMMARY:
The Community Care Coordinator (CCC) serves as a conduit between our clients and their business development staff, clinical staff and community referrals. The CCC is a clerical position with administrative and client promotional responsibilities.
QUALIFICATIONS:
- Education/Credentials: Preferred - Bachelor’s degree or healthcare/clinical setting experience.
- Experience: At least two years of administrative or sales experience.
- Core Competencies: Knowledge of medical terminology and regulations regarding medical records, confidentiality and HIPAA. Demonstrates excellent oral and written communication skills and attention to detail.
- Other: Valid driver's license, and auto insurance, reliable/well maintained transportation.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
1. Assists with placement, admission process and any other assigned tasks associated with service to patients and families.
2. Updates referrals, placement locations and others with status reports
3. Reports to management on various clinical and administrative matters.
4. Maintains continuing education through academic studies, seminars, and workshops.
5. Attends and participates in in-services, staff meetings and case conferences.
6. Completes all documentation in an accurate and legible manner.
7. Complies with company and clients’ policies and procedures, laws and regulations and standards of practice.
8. Other tasks, duties, projects and responsibilities as may be assigned.
Salary : $23 - $25
