JOB TITLE: Behavioral Health Care Coordinator
SUPERVISED BY: Director of Behavioral Health
STATUS: Hourly/Full-Time – Non-Exempt

Position Purpose
Under direct employment with Via Care Community Health Center, the full-time position is at one of our East LA locations. The Behavioral Health Care Coordinator will be based at East Los Angeles working directly with the Behavioral Health (BH) Director, Behavioral Health Clinicians, Via Care staff and providers, and the behavioral health patients. As a Behavioral Health Care Coordinator, you will be responsible for performing a variety of clerical functions to ensure an efficient operation and to enhance patient experience. This role would be responsible for scheduling initial and follow-up visits with a Behavioral Health provider, reminder appointment calls, insurance eligibility, Behavioral Health referrals, patient check-in and check-out and other related duties.
Essential Duties & Responsibilities
· Promote timely access to Behavioral Health (BH) care.
· Support the BH clinicians in serving those that reach out to Via Care for BH care.
· Maintain close communication with the Via Care call center.
· Obtain and address all new BH referrals through the Electronic Health Record system, and schedule all patients accordingly.
· Assist the BH clinicians in care coordination with patient appointments as clinically appropriate.
· Ensure all appointments are scheduled appropriately for both in-person and telehealth visits.
· Ensure all appointments are registered appropriately on the Electronic Health Record system; including patient registration, patient and insurance eligibility information, correct visit types, and other information needed.
· Document all outreaches and contacts that are conducted on the Electronic Health Record system.
· Troubleshoot patient issues.
· Act as the point of contact for all BH patients.
· Act as the point of contact for the BH clinicians regarding patient bookings.
· Maintain ongoing consistent communication with the BH clinicians.
· Develop and maintain patient waitlists as appropriate.
· Participation in monthly BH Department Quality Care meetings.
· Maintain confidentiality at all times.
· Perform other tasks as assigned.
Qualifications
Requirements
1. A minimum of two (2) years of related experience or a bachelor’s degree is required.
2. Experience in health or behavioral health-based social services setting working with clients who have social, psychological problems, chronic medical conditions, and homelessness or at risk of homelessness and/or young adults with serious mental illness or substance use disorders.
3. Current California driver’s license, car insurance, and a driving record acceptable to the Agency’s insurance carrier.
4. Current knowledge of all job specific skills including crisis intervention and multicultural and socio-economic issues, and documentation standards.
5. Knowledge of Agency policies and procedures, HIPAA, policies and documentation guidelines, and county, state and federal regulations relating to service programs for individuals and families with a history of mental illness and/or substance use.
6. Knowledgeable about and actively supports integrated mental health, physical health and substance use treatment for improving care for the whole person.
7. Basic advocacy skills to include a holistic approach to integrated health care and case management.
8. Skills to present ideas, information, and viewpoints clearly, both verbally and in writing.
9. Ability to utilize data, outcomes, experience and judgment to make effective business and therapeutic decisions.
10. Demonstrate commitment to team objectives and the Via Care mission.
11. Adapt to changing needs by acquiring new skills and knowledge.
12. Data entry skills required. Proficient in utilizing Microsoft Word, Excel and Power Point.
13. Effective communication and customer service skills.
14. Strong desire to work as part of multi-disciplinary team including school staff, counselors, LCSW and another medical provider.
15. Bilingual in Spanish strongly preferred but required.
16. Demonstrated expertise/skills in working with the target population.
17. Must be able to travel to off site and satellite locations.
18. Handles the coordination of emergency or crisis situations.
