Overview
We are seeking a detail-oriented and organized Admissions/Intake Coordinator to join our Home Health team. The Admissions/Intake Coordinator will play a crucial role in managing patient intake processes, verifying patient insurance eligibility, scheduling admissions, maintaining accurate medical records and providing exceptional service to patients and their families.
The Intake Coordinator is responsible for the overall coordination of referrals and scheduling for Medicare and non-Medicare clients.
- Receive referrals from physicians, facilities and staff and enter referral in DSL EMR.
- Ensure all demographic information is accurate on the referral form based on Medicare Eligibility.
- Assist with staffing/scheduling admissions
- Work closely with other members of the team to ensure adequate coordination and transition occurs to home health services.
- Our business is 24/7, so our office staff shares the handling of after hours on-call duties.
Responsibilities
- Manage the intake process for new and returning patients, ensuring all necessary information is collected accurately.
- Verify patient insurance information and eligibility, including Medicare coverage.
- Maintain and update medical records in compliance with HIPAA regulations.
- Collaborate with healthcare providers to facilitate patient care and address any inquiries or concerns.
- Provide exceptional customer service to patients, addressing their needs and concerns promptly.
Qualifications
- Proven experience in a medical office setting with a strong understanding of medical records management.
- Knowledge of Medicare processes and regulations is a plus.
- Strong organizational skills with attention to detail to ensure accuracy in patient information.
- Excellent communication skills, both verbal and written, to effectively interact with patients and healthcare professionals.
- Ability to work independently as well as part of a team in a fast-paced environment.
- Proficiency in using medical office management systems; Microsoft Office;
- Understanding of HIPAA regulations to maintain patient confidentiality.
