We are looking for a highly organized and detail-oriented Home Health Intake Coordinator to join our team. This role is essential in ensuring seamless coordination between patients, physicians, and clinical staff.
Job Summary
The Home Health Intake Coordinator is responsible for managing patient referrals, verifying eligibility, coordinating admissions, and supporting case management tasks. This role requires strong communication skills, medical knowledge, and the ability to multitask efficiently. The ideal candidate will have a background in healthcare and a passion for ensuring patients receive timely and appropriate care.
Key ResponsibilitiesIntake Coordination & Case Management:
- Process and review incoming referrals from physicians, hospitals, and other healthcare providers.
- Verify patient eligibility, insurance coverage, and authorization requirements.
- Gather and document necessary medical records and physician orders for admission.
- Communicate with patients and families regarding services, expectations, and care plans.
- Coordinate with nurses, therapists, and caregivers to ensure timely scheduling of assessments and visits.
Office Administration & Support:
- Maintain accurate and organized patient records in the electronic medical record (EMR) system.
- Assist with scheduling and coordinating home health visits.
- Ensure compliance with Medicare, Medicaid, and private insurance guidelines.
- Collaborate with the clinical team to ensure smooth operations and high-quality patient care.
- Address and resolve any intake-related concerns with referral sources and patients.
Qualifications & Requirements
- Education: Medical or healthcare background preferred (e.g., RN, LVN, LPN, Medical Assistant, or Bachelor's degree in Healthcare Administration, Nursing, or related field).
- Experience: At least 1-2 years of experience in home health, healthcare intake, or case management.
- Skills:
- Strong understanding of home health regulations and insurance requirements.
- Excellent organizational and multitasking skills.
- Strong communication and customer service skills.
- Proficiency in EMR systems and Microsoft Office (Word, Excel, Outlook).
- Ability to work independently and collaboratively in a fast-paced environment.
