Job Summary:
The Home Health Intake Coordinator plays a crucial role in the admission process by managing referrals, verifying patient eligibility, and coordinating services with healthcare providers and patients. This position requires strong communication skills, attention to detail, and the ability to work in a fast-paced environment while ensuring compliance with all healthcare regulations.
Key Responsibilities:
- Referral Management: Receive and process incoming home health referrals from hospitals, physicians, case managers, and other healthcare professionals.
- Patient Eligibility Verification: Confirm insurance coverage, benefits, and authorization requirements for home health services.
- Coordination & Communication: Serve as the primary point of contact between patients, families, caregivers, nurses (RN's, and LVN's), shower aid's, physical therapy providers, and medical professionals to ensure a seamless admission process.
- Data Entry & Documentation: Accurately enter patient information into the electronic medical record (EMR) systems and ensure all required paperwork is complete.
- Compliance & Regulations: Ensure adherence to Medicare, and private insurance guidelines, as well as company policies and HIPAA regulations.
- Scheduling & Coordination: Work closely with the clinical team to schedule initial patient visits and ensure a smooth transition into home health care.
- Customer Service: Provide compassionate and professional communication with patients and their families, addressing any concerns during the intake process.
Qualifications:
- Education: High school diploma or equivalent required; associate’s or bachelor’s degree in healthcare administration, nursing, or a related field preferred.
- Experience: Previous experience in home health, hospice, or healthcare intake is a must!
- Skills & Competencies:
- Strong organizational and multitasking abilities.
- Excellent verbal and written communication skills.
- Knowledge of medical terminology and insurance verification.
- Proficiency in EMR systems (Synergy, DSL, and Axxess), eFax systems, Google Workspace, and Microsoft Office Suite.
- Ability to work in a fast-paced environment with attention to detail.
Benefits:

- Pay: $25 to $30 USD per hour, based on previous experience!
- Full Time job, Monday-Friday, from 9am till 5pm, 8 hours per day.
