Weekend Referral Coordinator in our Reseda office
$21-$25 per hour range
Friday, Saturday and Sundays (8am-6pm)
Must be proficient in Microsoft Word, Excel, SharePoint, Outlook have excellent customer service phone skills, a multitasker in a fast-paced environment
Must be familiar with end of life/speaking with referral sources for hospice/palliative care services
Work independently and self-motivated
Handle a high volume of phone calls, emails, Teams messages
POSITION SUMMARY:
QUALIFICATIONS:
- Education in healthcare related field.
- Certified, licensed, or registered as appropriate.
- Teo years of healthcare experience required. One year experience in hospice preferred.
- Knowledge of hospice requirements including Medicare, Medicaid, and private insurance guidelines.
- Good communications skills.
- Ability to respond to the needs of physicians, discharge planners, patients and family members.
- Ability to coordinate services with referral sources, hospice staff and patients/families.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
- Addresses inquiries and requests for hospice information.
- Responds to referrals for evaluation and/or admission to hospice program.
- Verifies and completes all admission and financial information and conducts chart audits.
- Assures there is a valid physician order for assessment and admission.
- Confers with clinical management regarding admission decisions.
- Coordinates admission process between all team members.
- Explain the hospice/palliative program to patients, family members, and other referral sources in an accurate, complete and thorough manner.
- Provides statistical information to the Administrator as requested.
- Assures efficient transition of patient unto hospice service, including but not limited coordination of equipment, supplies and transport.
- Maintains continuing education through academic studies, seminars, and workshops.
- Attends and participates in in-services, staff meetings and case conferences.
- Completes all documentation in an accurate and legible manner.
- Complies with hospice policies and procedures, laws and regulations and standards of practice.
- Performs other tasks as may be assigned.
