Reporting to the Lead Case Manager, the Unaccompanied Children (UC) Foster Care Case Manager will be responsible for providing casework and supportive services to children in foster care, which includes comprehensive case management services, bi-weekly home visits, communication with identified family members both in home country and the U.S., and the identification and vetting of potential sponsors. In addition to programmatic responsibilities, the Case Manager will also be required to maintain communication with stakeholders in an effective and timely manner to ensure case continuity. Communication includes but is not limited to, email and phone communication with external stakeholders, case presentation during weekly staffing calls, scheduling of legal presentations, and collaboration with home study providers. This position is expected to function effectively with moderate supervision while following the guidelines given on procedures, along with the agency, state, and federal regulatory requirements. The position is primary on-site with the ability to work a hybrid schedule.

DUTIES

  • Ensure that all assigned youth on your caseload and under your supervision receive mandatory services while in care to include but not limited to mental health, medical, recreational, educational, and other supportive services in accordance with the time and documentation expectations of the program.
  • Develop and complete assessments, service and treatment plans, progress notes, incident reports, and any other necessary documentation in a timely manner.
  • Create a trusting and safe relationship with the client and encourage the personal growth of the client.
  • Maintain accurate and current information on children in care, while ensuring program's internal documentation practices and reporting requirements are met.
  • Actively participate in weekly staff meetings with the clinical team and stakeholders.
  • Attend, facilitate, and assist with providing ongoing education and support services to foster parents.
  • Attend weekly meetings with supervisor to consult on cases, review case plans and direction, and evaluate client participation and progress.
  • Schedule office, in-home and/or offsite meetings with assigned foster parents and children on a bi-weekly basis to address any concerns in service delivery.
  • Maintain effective communication with community agencies or individuals involved in service provision.
  • Attend internal and external training and apply newly gained knowledge in providing services.
  • Attend and prepare for required court hearings (if applicable).
  • Transport youth to appointments and unifications as needed.
  • Stay abreast of all agency, federal, and state regulatory requirements related to service provision.
  • Knowledge of and adherence to federal, state, program, and agency regulations.
  • Be able to work independently within the field.
  • Communicate effectively with supervisor, administrators, and all stakeholders.
  • Participate in the on-call rotation, which includes working after hours and weekends.
  • Complete other duties as assigned.

QUALIFICATIONS

  • Master's degree in social work; licensure in the state of Maryland (LMSW / LCSW / LCSW-C).
  • Experience in child welfare/case management strongly preferred.
  • Fluency in Spanish, required.
  • Prior experience working with refugee and/or immigrant populations, strongly preferred.
  • Excellent verbal and written communication skills, including interpersonal communication, group leadership skills, public speaking and clinical and persuasive writing skills.
  • Active motor vehicle license, clean driving record, and a reliable automobile with adequate insurance coverage.
  • Ability to pass a criminal history screen, including state and local child protection agency registries.
  • Up to 25% travel required including escorting youth to family unifications by car and air. Typically requiring same day travel but may, on occasion, involve overnight stays.

SPECIAL POSITION REQUIREMENTS

  • Must be willing and able to clear a criminal background check for the past seven (7) years.
  • Must be willing and able to request and clear Child Abuse and Neglect background check in all states lived within previous five (5) years, updated yearly.
  • Must be willing and able to obtain FBI fingerprint check every five (5) years.
  • Must have valid driver's license and MVA check free of major infractions for the last seven (7) years.
  • Must clear a State of Maryland Department of Public Safety and Corrections Services screening.
  • Must complete a bi-annual physical exam and TB testing, in accordance with Maryland state licensing requirements.
  • Must maintain proof of childhood and annual vaccinations, including: MMR, varicella, Tdap, and influenza.
  • Must be CPR, First Aid, and AEC certified upon hire or within 60 days of hire.
  • Must comply with an enhanced background check post-hire.
