Community Support Coordinator

We strive to assist our Participants in finding a home and being able to maintain their independance in thier residence or as they prepare to tranisition to thier own residence by completeing independant living assessments and teaching those skills that they are deficiant in. It is the responsibility of GOALS employees to assist our Participants in housing exploration, skill assessment/development, and job development in the areas we serve in Central Wisconsin. At GOALS we engage our Participants in community living by quality-of-life planning for individuals with disabilities. We work with referral agencies on behalf of Participants to complete independent living assessments and skill building for people with disabilities or other barriers to community living.

This position will be anticipated at this time to cover the Wood County area. There will be daily travel anticipated as well.

Prior experience in coordination of services with community agencies, preferred; however, will train the right motivated candidate. Valid driver’s license must have good driving record and insured vehicle available for daily use.

Duties will include case management including scheduling your workload and prioritizing, report writing, living assessments, collaboration with other community agencies, teach needed skills for the consumer to successfully complete tasks, and effectively communicating with your assigned work teams.

Job Type: Full-time

Pay: $13.50 - $14.50 per hour
