JOB DESCRIPTION
In-House Skilled Nursing Facility – PHYSICAL THERAPIST
This is a full-time hourly position aimed at 40 hours / week with a Monday – Friday schedule. No weekends are required.
Generally flexible on the hours for start / end times based on accommodating the needs of the patients and facility.
SUMMARY
Become a part of this in-house therapy program at this 90 bed skilled nursing facility as a staff Physical Therapist . The Physical Therapist is responsible for evaluating and treating patients for individualized Physical Therapy services in order to achieve the highest level of functional outcomes and overall patient satisfaction. On-site training and guidance will be provided to ensure success in the role!
ESSENTIAL DUTIES AND RESPONSIBILITIES
Providing individualized Physical Therapy services based on the patient’s unique clinical needs with courtesy and respect
Evaluating and treating patients with a wide range of medical conditions
Daily schedule will typically include 8-10 patients for a regular 8-hour day
Total Physical Therapy caseload ranges from 12-18 patients (shared with the Physical Therapy Assistant / PRN therapists as needed ).
Approx. 60-70% of the Physical Therapy caseload is short term with community discharge as the goal
Supervising 1 full time Physical Therapy Assistant and PRN as needed
Completing all the necessary documentation and billing in the EMR associated with providing Physical Therapy ( NetHealth for therapy and PointClickCare for nursing)
Completing administrative tasks relevant to providing therapy as needed (insurance updates, durable medical equipment paperwork, etc.)
Conduct screens for therapy services to ensure long term care resident needs are met
Providing educational in-service trainings as needed to nursing staff
Collaborating with all members of the therapy team (Physical, Occupational, and Speech) to provide the highest degree of care
Serving as a patient advocate for therapy services
Communicating with family with compassion
Actively participating in therapy team and facility meetings
Caseload review and development
Discharge planning
Patient careplans with families
QUALIFICATIONS
Current and in good standing licensure as a Physical Therapist in the state of IL
Current CPR certification
Experience in Long Term Care is preferred but not required.
