ADDICTION RECOVERY CASE MANAGER
Goochland Powhatan Community Services is committed to helping individuals achieve long-term recovery from substance use disorders. We are seeking an experienced Addiction Recovery Case Manager to join our team and provide critical support services to clients and families.
In this role, you will work closely with clients, family members, and healthcare providers to assess needs, develop strategies, and monitor progress. Your duties will include conducting psychosocial assessments, creating individualized service plans, monitoring treatment compliance, and collaborating with community partners to ensure seamless coordination of care.
To succeed in this role, you must possess a deep understanding of addiction recovery principles and practices, as well as excellent communication and interpersonal skills. A Bachelor's degree and relevant experience are required. CSAC certification is highly desirable, and familiarity with MAT for opioid dependence is an asset.
