What are the responsibilities and job description for the Clinical Liaison position at Good Samaritan Hospice?
Good Samaritan Hospice is seeking a full-time Clinical Liaison to work closely with our partners along Route 28.
The Clinical Liaison is responsible to coordinate all offered services for home healthcare as applicable. This may include completing part or all of the admission process for patients. The Clinical Liaison partners with the sales team in developing and maintaining referral source relationships. Their main responsibility is discharge planning to the agency from facilities.
QUALIFICATIONS for the Clinical Liaison position:
- Associate's degree in nursing recommended.
- Case manager certification and/or specialty certification recommended.
- Maintains current PA Driver’s License and auto insurance.
- Strong clinical assessment skills.
- Skills in typing and organizational abilities.
- Preferably has home healthcare experience.
- Able to work independently, demonstrating good judgment.
- Must be accurate and detail oriented.
- Practices good customer service skills in all working relationships.
- Desire and ability to learn.
- Strong interpersonal skills, respectful, and courteous nature.
- Absence of criminal background.
