Essential Duties and Responsibilities:

The Recuperative Care Case Manager position will provide support and assistance to individuals who are experiencing homelessness or are at risk of becoming homeless. The primary focus of a recuperative care case manager is to help these individuals with acute medical issues access and receive appropriate medical care and other essential services during their recovery period. The program aims to reduce the overutilization of emergency and inpatient medical services by

providing shelter, case management and advocacy for person experiencing both homelessness and

medical issues. To increase access to care and reduce hospital readmissions, leading to successful

community reintegration.

  • Maintaining a full census
  • Transportation from hospital or referral source to shelter
  • Assess client for program
  • Responsible for entering client into ETO, HMIS and CES
  • Transportation to patient health and wellness education
  • Accompany clients to appointments as necessary; advocate with medical professionals to ensure
  • care continuity and client understanding and follow through
  • Medication compliance
  • Schedules no less than one hour weekly with client to assess medical and social needs
  • Creates care plan with client which addresses any and all medical issues per referral source
  • and/or any needs the client wishes to address
  • Educates client how to access medical care and address issues without unnecessarily utilizing
  • emergency medical services
  • Coordinate with Home Health agencies to implement discharge plan and follow up services
  • Assist and Advocate for client in accessing additional health services, including primary and
  • specialty care services
  • Nutritional education and wellness support activities/ information provided as medically
  • appropriate
  • Reviews case plans with Program Manager
  • Maintains condition of recuperative care area and enforces rules/policies of shelter and
  • program
  • Will note in ETO every interaction with or for the client such as speaking with the client, making

a phone on behalf the client

  • Ensure that all clients have their ID, birth certificate, proof of SSDI or SSI, social security card, and
  • 6 months of bank statements; for document ready Document ready should occur within first 30-
  • 60 days
  • Refer clients to services available within the community that meet specific needs or

support progress

  • Submit CenCal authorization for RCP services
  • Submit ECM authorization
  • Maintain high ethical standards in all aspects of the role. Adhere to professional codes of conduct, respect client confidentiality, and prioritize the well-being and rights of clients at all times.
  • Be prepared to respond to crisis situations promptly and effectively, providing immediate assistance and access to appropriate resources when needed.
  • Responsible for promptly notifying the manager of any work orders that need to be submitted or supplies that need to be ordered.
  • Responsible for attending staff meetings, ensuring timely completion of training, and accurately completing timecards.
  • Perform related duties as assigned by the supervisor.
  • Maintain compliance with all company policies and procedures.

Education and/or Work Experience Requirements:

  • High School Diploma or equivalent: A minimum educational requirement to demonstrate basic literacy and communication skills.
  • Associate degree preferred.
  • Must hold a CNA (Certified Nursing Assistant), MA (Medical Assistant), or EMT (Emergency Medical Technician) certification.
  • Minimum one year experience in a homeless services setting working directly with individuals experiencing homelessness or substance abuse.
  • Familiarity with available resources to support homeless clients is essential.
  • Excellent communication skills: Ability to effectively communicate with shelter residents, colleagues, and external stakeholders.
  • Crisis intervention skills: Capacity to handle crisis situations and de-escalate conflicts.
  • Empathy and compassion: Demonstrating genuine care and understanding for individuals experiencing homelessness.
  • Ability to work in a team: Collaboration and cooperation with other shelter staff, volunteers, and external agencies.
  • Excellent computer proficiency (MS Office – Word, Excel and Google)

Required Qualifications:

Employment Eligibility Verification:

  • Must provide documentation to establish identity and employment eligibility in compliance with legal requirements. This may include a U.S. passport, Permanent Resident Card, or other acceptable forms of identification.

Negative Tuberculosis Test:

  • Must provide documentation of a recent negative tuberculosis (TB) test result to ensure a safe and healthy work environment.

Background Screening:

  • Successful completion of a comprehensive background screening, which may include criminal record checks, employment history verification, and reference checks.

Pre-Employment Drug Screening:

  • Must pass a pre-employment drug screening to ensure a drug-free workplace. A negative drug test result is required.

Driving Skills:

  • Possess strong driving skills and have a valid driver's license.
  • Driving is an essential function of the position, and a clean driving record may be required.
  • Must possess a valid California driver's license or obtain one before the start of employment.

  • Must be able to qualify for Good Samaritan Shelter insurance coverage, which may include meeting specific criteria related to driving record, insurance claims history, and other factors.

Personal Insurance Coverage:

  • Must have personal insurance coverage that meets the requirements set by the employer.
