Job Responsibilities:
- Utilize the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care in the home setting.
- Collaborate with other inter-professional colleagues, to plan, implement and evaluate care.
- Documents resident history, symptoms, medications and care provided in compliance with applicable standards/regulations.
- Utilizes technology to enhance quality resident care, to collaborate with inter-professional team members, effectively delegate to and supervise unlicensed personnel, assume overall nursing accountability for a resident assignment and serve in leadership roles on department/organizational tasks forces and committees.
Requirements:
- Bachelor's degree in nursing preferred.
- Minimum of two years' long term care or geriatric nursing experience preferred.
- Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
- Obtains and subsequently maintains required department specific competencies and certifications.
