Job Summary
The Clinical Care Leader (CCL) is responsible for utilizing the nursing process to provide individualized nursing care in the home setting. The CCL collaborates with other inter-professional colleagues to plan, implement, and evaluate care.
The CCL documents resident history, symptoms, medications, and care provided in compliance with applicable standards and regulations. The CCL utilizes technology to enhance quality resident care and collaborate with inter-professional team members.
The CCL assumes overall nursing accountability for a resident assignment and serves in leadership roles on department/organizational task forces and committees.
The CCL must have effective communication skills to collaborate, negotiate, and resolve conflicts, situations, and/or issues with peers, inter-professional colleagues, residents, and families.
The CCL must have strong clinical reasoning and critical thinking skills to analyze data and respond to changing situations.
Requirements
A Bachelor's degree in nursing is preferred. A graduate from a nationally accredited nursing program is also preferred.
A minimum of two years' long-term care or geriatric nursing experience is preferred. Minimum Data Set (MDS) experience is also preferred but not required.
The CCL must hold an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
