Responsibilities
- Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
- Collaborate with interdisciplinary teams to develop and implement comprehensive care plans that meet the unique needs of each resident.
- Communicate effectively with residents, families, and healthcare providers to ensure seamless transitions and optimal outcomes.
Qualifications
- Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
- Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges.
