As a Patient Wellbeing Coordinator, you will play a critical role in coordinating patient care, ensuring seamless transitions, and supporting the delivery of high-quality services.

Job Description:

We are seeking an experienced Registered Nurse to join our team as a Patient Wellbeing Coordinator. In this role, you will work collaboratively with other healthcare professionals to coordinate patient care, address resident concerns, and promote the overall wellbeing of our residents.

Responsibilities:

• Coordinate patient care, including assessments, care planning, and implementation.
• Collaborate with interdisciplinary teams to ensure continuity of services and optimal patient outcomes.
• Communicate effectively with patients, families, and colleagues, using empathetic responses and active listening skills.

Requirements:

• Current RN licensure with the State Board of Nursing or multistate licensure if in a Nurse Licensure Compact (NLC) state.
• Graduate from a nationally accredited nursing program preferred.
• Strong communication and interpersonal skills, with ability to work effectively in a fast-paced environment.
