Job Overview
We are seeking a dedicated and detail-oriented MDS Coordinator to join our healthcare team. The ideal candidate will play a crucial role in ensuring compliance with regulatory requirements and maintaining high standards of patient care. This position involves working closely with clinical staff to assess patient needs and coordinate care plans, particularly for those in critical care, inpatient, outpatient, and home care settings. Familiarity with medical coding systems, including ICD-10 is essential for this role. Also beneficial to this role is experience with Matrix and Quick MAR through Point Click Care.
Responsibilities
- Conduct comprehensive assessments of patients to determine their medical needs and eligibility for services.
- Collaborate with healthcare professionals to develop and implement individualized care plans.
- Ensure accurate documentation of assessments and care plans in the electronic health record system, preferably Cerner.
- Monitor patient progress and adjust care plans as necessary based on clinical evaluations.
- Maintain compliance with federal and state regulations related to MDS documentation and reporting.
- Provide training and support to staff on MDS processes and documentation requirements.
- Participate in quality assurance initiatives to improve patient outcomes.
- Stay updated on changes in regulations, coding practices, and healthcare trends relevant to MDS coordination.
Qualifications
- Bachelor’s degree in Nursing or a related field; RN license preferred.
- Proficiency in medical coding systems, specifically ICD-10.
- Familiarity with Matrix and Quick MAR or similar electronic health record systems is a plus.
- Strong analytical skills with attention to detail for accurate documentation.
- Excellent communication skills to effectively collaborate with interdisciplinary teams.
- Ability to work independently while managing multiple priorities in a fast-paced environment.
- Previous experience as an MDS Coordinator or similar role is preferred but not required.
