Title : CASE WORKER
Reports To : SUPERVISOR
Location : 2471 Morris Ave. Bronx, NY 10468
Hours : 35 hours, exempt
Salary : $50,000-$55,000 / year
Job Summary : The Family Counseling for School Success Program provides counseling and case management services to families connected with PS / MS15, PS291, PS 246 or live within the community. The Caseworker will provide strength-based counseling and case management services to assigned families utilizing Motivational Interviewing and Solution-Based Casework (SBC) Model.
Major Duties :
Case Practice Responsibilities :
Conducts outreach to all assigned referrals within 48 hours of assignment.
Schedules to meet with families within 7 days to assess for services.
Provide ongoing individual and family counseling to children and families.
Provide short-term crisis intervention.
Conduct home visits.
Collaborate with the family worker to provide concrete needs assessment and referrals when indicated.
Creates a productive partnership with contracted schools on families involved in counseling.
Ongoing collaboration with providers connected to the families.
Assess and refer families to program parenting group and program events, when indicated.
Participate in agency events for participants.
Administrative Responsibilities :
Actively participate in weekly supervision to develop counseling and case management skills.
Attend and participate in regular case consultation, unit meeting and staff meetings.
Maintain a regular caseload in accordance with the program’s policies and procedures.
Complete all required ACS and GSS trainings.
Prepare appropriate statistical reports (PROMIS).
Complete progress notes within 5 business days of event.
Service / Family plans to be completed by due dates.
Implement and integrate the principles and procedures of Solution Based Casework : assessment, family and individual level outcomes, action plans and documentation and celebration.
Work towards SBC certification; and once certified continue to implement and document SBC practices.
Perform other duties as assigned.

Qualifications :
BSW / BA preferred; will consider a high-school diploma with related experience navigating child welfare and / or school systems.
Bilingual (English / Spanish) preferred.
Must be able to work two evenings per week until 7 : 00 p.m.
Excellent organizational, written, and communication skills.
Must be proficient in Microsoft Office
