Job Responsibilities

Provide high-quality post-acute care according to resident care plans and best practices

Collaborate with the nursing and clinical team

Assist RNs with tests and medical procedures

Measure and record residents' vital signs

Administer medications and vaccinations

Maintain accurate medical records while ensuring confidentiality

Communicate residents’ concerns to the staff

Build trust and relationships with residents and family members

Assist in training nursing and non-nursing staff

Promote a safe and inclusive environment

Qualifications and Skills

Valid and unencumbered Ohio LPN license

Graduate from an accredited nursing program

Self-starter with the ability to work independently

Reliable transportation required
