Job Responsibilities

Deliver high-quality post-acute care in accordance with resident care plans, nursing best practices, facility policies, and legal requirements.

Coordinate care with the nursing and clinical team.

Assist RNs and clinicians with medical tests and procedures.

Measure and record residents' vital signs and their physical and mental conditions.

Administer medications and vaccinations as directed.

Obtain resident medical histories and ensure medical records are current, complete, and confidential.

Listen to residents and communicate their concerns to staff.

Engage with residents and family members to foster trust, build relationships, and enhance resident satisfaction.

Assist in training nursing and non-nursing support staff.

Promote a safe and inclusive environment for residents and fellow staff members.

Perform additional duties as assigned by leadership.

Qualifications and Skills

Active and unencumbered Ohio LPN license.

Graduation from an accredited nursing program.

Ability to take initiative as a self-starter and work independently and efficiently.

Must have reliable transportation.
