What will the job look like?
Under the direction of the Program Manager, the primary responsibility of the Case Manager is to work one-on-one with clients to ensure they are provided with the resources, training, and support to help them with climate employment opportunities. Responsibilities will include client intake and assessment, development of an Individualized Plan and actionable goals, coaching, training, advocacy, referrals, providing resources, appropriate training recommendations, case noting, and consulting with internal and external support personnel, among others. This position will also have responsibility for an assigned scope of program services, that will include ensuring program goals are met and that fiscal and grant outcomes are achieved.
What are the essential responsibilities?
- Problem solve with participants to resolve barriers to success, maintain knowledge of community resources.
- Assist individuals to pursue and achieve employment and educational goals, as applicable, and accessing internal and external resources.
- Document all activities and participant progress through their individual Employment Plan or Individual Goal Plan in files and document in the CaseWorthy system.
- Ability to establish a professional working relationship with all levels of internal and external staff and participants.
- Provide guidance and coaching to clients as needed.
- Serve as a liaison between the participant, agency, and the business and establish and maintain working relationships with professionals and business persons within the community.
- Develop and maintain relationships with agencies and other outreach activities to increase referrals for enrollment and participant recruitment. Provide and document appropriate participant referrals.
- Coordinate and facilitate assigned company and agency events and workshops, including communication and marketing needs.
- Assist participants with recommended skills training as needed, including general computer skills and Microsoft Office and Google training as needed.
- Maintain files on participants to ensure accurate and timely data entry in compliance with CARF and agency/funders guidelines.
- Possess excellent customer service skills, communication skills, including excellent active listening and speaking skills. Must be a self-starter and good team player.
- Participate in meetings, trainings, conferences, and special events as necessary. Participate in audit and monitoring visits as required.
- Provide own transportation as required, with ability to travel within designated service area as needed.
- Perform all other related duties as assigned.
