What are the responsibilities and job description for the Case Manager Job Coach position at GOODWILL INDUSTRIES OF GREATER DETROIT & SUBSIDIARIES?
Position Duties : 1. Research, study, and apply principles from the strengthens-based case management model during daily interactions with participants. 2. Maintain current working knowledge of federal, state, and local barrier removal programs. 3. Coordinate and facilitate participant services such as program intake, assessments, evaluation, planning, and implementation, including goal setting, crisis intervention, and barrier removal. 4. Review participants referral documents to understand psychological backgrounds / diagnosis, criminal history, behavior trends, probation requirements and other risk factors. 5. Develop Individual Service Plans (ISP's) in collaboration with participants to determine self-selected programmatic goals and service tracks. 6. Monitor each assigned participants progress towards ISP goal attainment and provide guidance throughout the process of personal transformation. 7. Identify assigned participants barriers to achieving ISP goals and submit internal / external supportive services request for barrier removal purposes. 8. Provide regular and timely input of detailed participant case notes into the agency's customer relationship management system(s) for persons served giving an accurate account of individual services provided, incidents, intervention, follow-up, and progress reporting. 9. Manage case records in accordance with agency policies and procedures including CARF accreditation standards, funding source requirements and audit compliance regulations. 10. Ensure compliance with HIPPA, confidentially laws and established internal quality control procedures for records maintenance, reports, service delivery, operating practices, and documentation in preparation for internal and external auditing, accreditation, and other appropriate regulatory bodies. 11. Review and analyze monthly, assigned caseload demographics, the active vs inactive caseload ratio, supportive service submission approvals, external resource referrals, ISP progress, classroom attendance, job retention requested interventions vs. inactive case counts, mental health service enrollments, termination request and other outcome metrics. 12. Assist program participants with understanding their workplace readiness needs, developing their career interest, workplace aptitude, creating a resume, mock interviews and provide job placement services. 13. Provide post-placement retention services such as job coaching, career pathway matriculation strategies, collection of check stubs, motivation to pursue career options through job training and encouragement to engage in GED preparation classes, post-secondary education, and financial coaching sessions. 14. Compile and submit participant milestones or accomplishments, or standard case management report of Progress report for participant 15. Perform other reasonable assignments as required.
