Job Title : Telephonic Nurse Case Manager

Job Overview :

We are seeking a compassionate and experienced Telephonic Nurse Case Manager to join our innovative case management team. The Telephonic Nurse Case Manager will be responsible for providing telephonic care coordination, case management, and clinical support to patients with complex health needs. This position involves working with patients over the phone to assess their needs, develop care plans, and ensure they receive appropriate medical care and services. The ideal candidate will have strong clinical skills, experience in telephonic case management, and the ability to effectively manage patient cases remotely. This position is remote, with occasional in-person meetings or field visits as required.

Key Responsibilities :

Patient Assessment and Care Planning :

  • Conduct thorough telephonic assessments to evaluate patients" physical, emotional, and social needs.
  • Develop individualized care plans in collaboration with patients, their families, and the healthcare team.
  • Monitor patient progress and adjust care plans as needed based on ongoing assessments.
  • Ensure that all care plans align with patient goals and promote optimal health outcomes.

Telephonic Case Management and Care Coordination :

  • Provide telephonic case management for patients with chronic conditions, complex health needs, or recovering from acute illnesses.
  • Coordinate care with healthcare providers, specialists, and community resources to ensure patients receive timely and appropriate care.
  • Assist patients with navigating the healthcare system, including scheduling appointments, coordinating services, and accessing necessary medical treatments and resources.
  • Advocate for patients to ensure they receive timely access to care and services.
  • Patient Education and Support :

  • Empower patients to take an active role in managing their health by providing resources, information, and guidance.
  • Monitoring and Reporting :

  • Track patient progress and follow up regularly to assess the effectiveness of care plans.
  • Document patient interactions, care plans, and outcomes in compliance with regulatory and organizational standards.
  • Provide regular updates to management regarding patient outcomes and any challenges encountered.
  • Quality Improvement and Compliance :

  • Participate in quality improvement initiatives to enhance patient care and operational efficiency.
  • Ensure compliance with all applicable healthcare regulations, organizational policies, and best practices in case management.
  • Identify opportunities for improvement and contribute to the development of new care protocols or procedures.
  • Collaboration and Team Communication :

  • Work closely with physicians, social workers, lawyers and other healthcare providers to ensure coordinated, holistic care for patients.
  • Communicate regularly with other team members to discuss patient needs, updates, and strategies for improving care delivery.
  • Attend interdisciplinary team meetings as needed to collaborate on complex cases.
  • Qualifications :

  • Current RN or LPN license holder
  • Minimum 3 years of nursing experience, with at least 1-2 years in case management or a similar role, preferably with telephonic or remote case management experience.
  • Certification in Case Management (CCM) or other relevant certification preferred.
  • Strong knowledge of healthcare systems, case management processes, and community resources.
  • Familiarity with telehealth platforms and case management software.
  • Experience in managing patients with chronic conditions, acute care needs, or complex health issues.
  • Skills :

  • Excellent telephonic communication skills, with the ability to build rapport and trust with patients remotely.
  • Strong clinical assessment, critical thinking, and problem-solving abilities.
  • Ability to prioritize tasks and manage a caseload effectively.
  • High level of professionalism, confidentiality, and ethical conduct.
  • Strong organizational and time management skills.
  • Proficient in using electronic health records (EHR) and case management software.
  • Work Schedule :

  • Full-time, remote position with flexible hours.
  • Occasional telephonic meetings or visits to patient appointments with healthcare providers, as needed
  • No weekends or holidays required.
