Client Assessment:
- Conduct thorough in-home or field-based assessments to evaluate clients' medical, social, and emotional needs.
- Collaborate with clients, family members, and healthcare providers to develop and implement personalized care plans.
- Facilitate access to necessary medical services, community resources, and support systems for the well-being of clients who have been involved in auto accidents or work injuries.
- Stay informed of local healthcare services and community programs to ensure effective coordination of care.
- Provide nursing or therapeutic support, education, and emotional encouragement to clients and their families during in-home visits.
- Empower clients to make informed healthcare decisions through clear, compassionate communication.
- Regularly monitor clients' progress in the field and adjust care plans as necessary to ensure optimal health outcomes.
- Maintain accurate documentation of client interactions and health updates in compliance with organizational policies.
- Work closely with a variety of healthcare professionals, including physicians, social workers, and therapists, to provide comprehensive, client-centered care.
- Actively participate in team meetings to discuss client progress and ensure coordinated care efforts across disciplines.
- Advocate for clients' needs within the healthcare system and help navigate community resources to meet those needs.
- Educate clients and families on healthcare processes, rights, and available support services.
- Engage in ongoing training to stay up-to-date with the latest practices and case management strategies.
- Share best practices and insights with team members to enhance the quality of care provided to clients.
- Current licensure or credentials as an LPN, RN, CCM, CBIS, BSN, CLCP, CRRN, CLNC, MSW, OTR/L, or similar in the state of Michigan.
- Minimum 2 years of nursing, case management, or related experience preferred.
- CCM (Certified Case Manager) or CBIS (Certified Brain Injury Specialist) certification preferred.
- Knowledge of healthcare systems and community resources relevant to client care.
- Ability to work independently and manage time effectively in a field-based role.
- Excellent communication, interpersonal, and organizational skills.
- Proficient in using electronic health records (EHR) and case management software.
- Strong analytical and problem-solving abilities.
- Ability to maintain confidentiality and handle sensitive client information with professionalism.
- Exceptional time management skills, with the ability to manage a high volume of field-based visits.
- Fluency in additional languages is preferred.
- Required to work in the office 2 days per week; the remaining days will be spent in the field visiting clients.
