Essential Duties & Responsibilities
- Collaborate with TMs to determine their needs & priorities, & together design a plan of action.
- Gather & record TM demographic & referral information to enter into established Electronic Health Record (EHR) within 24 hours of initial contact.
- Accurately document all services provided to TMs in established EHR.
- Identify resources & refer to Goodwill Industries of the Inland Northwest (GIIN) & community resources to assist TMs in building independence & improving stability.
- Provide direct support, coaching, & mentoring to TMs to reduce barriers & assist with navigation of resources.
- Maintain up-to-date knowledge of community resources & develop relationships with local community organizations.
- Create individualized plans for TM requests, with specific plans & objectives.
- Practice cultural humility, while demonstrating the necessary attitudes, knowledge, & skills to deliver culturally competent services.
- Maintain confidentiality, utilizing Release of Information authorizations to facilitate services & advocate for TMs while communicating with collateral contacts as needed & authorized.
- Monitor & follow up with TMs as needed while cases remain active, closing cases monthly when all goals & objectives have been accomplished or at the request of TM.
- Maintain flexibility to work occasional evenings or weekend shifts, with supervisor approval.
- Travel between GIIN locations.
- Demonstrate respect & value for all TMs, regardless of circumstances, behavior, or needs.
- Participate in meetings, staffings, trainings, & other community activities as a positive & professional ambassador of GIIN.
- Establish & maintain positive, professional working relationships with Team Members, managers, GIIN leadership, & community partners & resources.
- Notify supervisor, law enforcement, & other appropriate entities regarding safety concerns or when required by Mandated Reporting statute or GIIN policy.
- Work independently & collaborate with GIIN department, site managers, & colleagues to promote GoodLife services & activities.
- Stay apprised of policy, state, & federal laws pertaining to GIIN & the individuals it serves.
- Participate in quality assurance, continuous improvement, outcome measures, & reporting systems.
- Track revenue & expenses.
- Provide services in accordance with GIIN policies, CARF accreditation standards, & principles of safety.
- Work collaboratively with Workforce & Family Services to provide participant services.
- Follow all policies, procedures, & directives of Goodwill Industries of the Inland Northwest assuring safety of personnel or property.
- Properly wear & maintain all required Personal Protective Equipment (PPE).
- Maintain agency confidentiality. Violation of confidentiality is cause for immediate dismissal.
- Comply with & promote good safety practices. Maintain an orderly work area. Comply with all health, safety, & fire standards & all local, state, & federal regulations (WISHA & OSHA).
- Other duties as assigned.
