- Primary Responsibilities:
This is a full-time, salaried position with fringe benefits.
- Provides the following services to referrals, walk-ins, and employees:
- Career coaching and services coordination for people with disabilities
- Employment services including employment planning, job development planning and placement services, and employment supports
Career Coaching and Services Coordination
- Perform intake and assessment to develop a customized plan based on the person’s strengths, abilities, preferences, and desired outcomes.
- Develop an individual career plan that identifies services/supports that are needed by the person to progress toward desired outcomes.
- Coordinate all necessary services/supports for the person being served. Referrals to other human service agencies or community resources will be made if deemed appropriate.
- Monitor the progress of each person concerning their career plan.
- Responsible for all follow-up activities to achieve the career plan.
- Function as a liaison with all community partners (and internally) to ensure individuals achieve the goals and objectives of their career plan.
- Assist a person seeking employment to learn about employment opportunities and to make informed decisions using some type of employment exploration model.

- Develop a written employment planning report based on information provided from the Job Coach evaluation.
- Maintain awareness of labor market trends, high demand, and high growth occupations, and job availability.
- Job development planning to assist a person in identifying, obtaining, and/or advancing in employment.
- Assistance at a basic level, such as support for self-directed job search.
- Assist employers to identify and eliminate barriers to employment for individuals placed.
- Negotiates wage subsidies and tax incentives with employers.
- Provide follow-up and support services to persons to assist them in adjusting to and maintaining their employment.

- Responsible for written documentation and accurate records in compliance with agency standards, CARF, and as identified by the funding source and/or referral source.
- Provide classroom instruction and/or training.
- Perform the job functions of a Job Coach in the community, as needed.
- Sedentary work, occasionally exerting up to 50 pounds of force and/or 25 pounds of lifting carrying, pushing, pulling, bending, reaching, handling, hearing, talking, and seeing.
- Inside is generally 100%, however, may include outside work on occasion.
- Moderate emotional level effort, occasional short deadlines requiring some overtime. The work environment is sometimes hectic with occasional high stress.
- Requires some travel within our regional area with personal vehicle or agency transportation.
- Bachelor’s degree in Human Services or a related field required; or equivalent combination of education and experience. Depending on the assignment, a program/grant may require an actual degree regardless of experience.
- A minimum of two (2) years of experience in case management or employment service programs is required.
- Good organizational skills. Strong communication skills, both oral and written.
- Must be able to provide positive internal and external customer relations and to easily interact and work with individuals referred for services.
- Work effectively with referral sources and case coordination staff to ensure the continuity of services.
- Proficient in the use of computers and Microsoft Office suite.
- Maintain a good driving record, valid insurance, valid driver's license.
