Job Summary
Responsible for providing community employment support for individuals with barriers to learn, accommodate, and perform their job duties so that they may become a successful employee. The On-Call Job Coach provides job site training, workplace, individualized, and follow up support services.

Responsible for providing community employment support for individuals with barriers to learn, accommodate, and perform their job duties so that they may become a successful employee. The On-Call Job Coach provides job site training, workplace, individualized, and follow up support services.
Responsibilities
Community Employment Support Services including:
- Provide side-by-side job coaching at employment sites for individuals in need of additional short term or long term employment supports.
- Complete task analysis of client job duties to develop training strategies prior to job-site training.
- Determine accommodations and design strategies that match the needs of the worker and the employer.
- Identify workplace supports and develop a Work Supports Plan to provide support that is needed.
- Provide training on the job skills. Teach appropriate social and soft skills on the job.
- Complete and communicate work evaluations to measure the worker's performance, work habits, and skills. Coach to improve performance.
- Collect and analyze data to document effectiveness of workplace supports and to support retention of the job site.
- Successfully performing new job duties that were assigned after job start.
- Relearning skills previously learned but being performed incorrectly or poorly.
- Problem solving issues occurring on and off the job that are impacting employment.
- Support the Job Placement Supervisor in the development of skill appropriate jobs in the community for client placement.
- Ensure timely and quality case noting in case management system. Responsible for written documentation and accurate records in compliance with agency standards, CARF, and as identified by the funding source and/or referral source.

Education
- GED/diploma required
- Associate's Degree in Human Services preferred
Experience
- 2 years of experience in Case Management or Employment Service Programming preferred
Certifications/Licenses
- Valid driver's license and insurance required
