POSITION SUMMARY:
Goshen Community Services (GCS) is seeking a dedicated and passionate Foster Care Case Management Specialist to join our team in Omaha. This full-time position plays a critical role in providing case management services to children and families under the supervision or custody of state agencies. The Foster Care Case Management Specialist is responsible for managing a caseload of youths enrolled in foster care services focusing on ensuring the permanency, safety, and well-being of each child. The FCCMS works closely with foster families, biological families, and other professionals to provide case management and coordinated services while advocating for the best interest of the child.
As a Foster Care Case Management Specialist, you will be responsible for supervising, supporting, and guiding foster parents who care for children with emotional and behavioral challenges. This includes conducting ongoing meetings with foster parents, developing individualized safety plans, and attending necessary care management meetings and court hearings. You will also assess the needs of children and families and provide ongoing support throughout the placement process.
This position requires strong written and verbal communication skills, the ability to work with a diverse population, and a genuine passion for helping children find stability and a loving family environment. As part of the GCS team, you will demonstrate a commitment to connecting children with families and ensuring that family connections are maintained.
SPECIFIC RESPONSIBILITIES:
Case Management:
- Develop, implement, and monitor individualized care plans for children in foster care.
- Conduct regular home visits to ensure the safety and well-being of the child.
- Document case activities, progress, and updates in compliance with state and agency regulations.
Foster Family Support:
- Provide ongoing support, resources, and training to foster families.
- Assist families in managing challenges and fostering a nurturing environment for children.
- Assist foster families with youth transportation to visits and appointments.
Child Advocacy:
- Act as a liaison between the child, foster family, biological family, and other stakeholders.
- Attend court hearings, school meetings, and medical appointments as needed.
- Advocate for the child’s physical, emotional, educational, and social needs.
Collaboration and Coordination:
- Collaborate with social workers, therapists, educators, and other professionals involved in the child’s care.
- Facilitate reunification efforts when appropriate or assist with the adoption process if reunification is not possible.
- Coordinate services such as counseling, therapy, and educational support.
Compliance and Reporting:
- Ensure all foster care placements and practices adhere to state regulations and agency policies.
- Complete and submit required documentation and reports in a timely manner.
- Stay informed about changes in foster care policies and best practices.
Support to Foster Parents:
- Provide support and guidance to foster parents throughout the placement process.
- Conduct regular visits to foster homes to assess the well-being of children and provide additional support as needed.
- Advocate for the needs of children and families to ensure they receive appropriate services and support.
Crisis Intervention:
- Respond to crisis situations involving placements to ensure the safety and well-being of children.
- Collaborate with other agencies and organizations to provide crisis intervention services to children and families in need.
Documentation and Reporting:
- Maintain accurate and up-to-date records of placements and related activities.
- Prepare reports on placement services for internal and external stakeholders as needed.
- Prepare and maintain monthly progress reports on childcare development and well-being.
Other Duties:
- Perform other duties as assigned by supervisors or management to support GCS’s programs and services.
SCHEDULE:
- 8-hour shift
- Monday to Friday
- On-Call Rotation
QUALIFICATIONS:
- Bachelor's degree in social work or a related field (Master's degree preferred).
- Minimum of 1 year of experience in child welfare or a related field.
- Knowledge of Nebraska 391 NAC regulations and experience in ensuring compliance.
- Strong communication and interpersonal skills to effectively engage with children, families, and stakeholders.
- Ability to work effectively in a team environment and collaborate with other staff and agencies.
