Description
The Stroke Program Coordinator facilitates program operations and patient management in collaboration with leadership and staff. The Coordinator also functions as an educator to the patient, the patient's family and hospital staff providing care to the Stroke patients.

The Stroke Program Coordinator facilitates program operations and patient management in collaboration with leadership and staff. The Coordinator also functions as an educator to the patient, the patient's family and hospital staff providing care to the Stroke patients.
Essential Duties
- Responsible for the Stroke Program development, quality assessment, improvement and inter/intra disciplinary communications
- Responsible for coordinating and leading all activities associated with the Stroke Program.
- Evaluates Stroke Program through the use of data capture, follow-up and trend analysis and presents findings to appropriate committees
- Monitor the planning, delivery and outcomes of care for the stroke patient population
- Collaborate with other health team members to provide coordinated, safe, and compassionate care for stroke patients.
- Actively participates in patient education validating competency levels in care of the stroke patient.
- Collects and enters data for the national stroke registry and research protocol.
- Prepares reports indicating progress and/or opportunity for improvement.
- Ensures that the hospital meets regulatory requirements for maintaining a stroke program.
- Coordinates and facilitates Stroke Committee
- Fosters the professional development of unit staff through support of the orientation process and other educational activities.
- Function as an expert nurse who provides leadership for the organization of services and systems necessary for a collaborative approach to stroke care.
- Assists with the development and implementation of initiatives, policies and procedures, including staff education activities related to stroke care
- Serves as the liaison between the hospital and the community in providing education related to the stroke program.
- Uses critical thinking skills, professional judgment, and leadership, to anticipate, plan, direct and evaluate the care of stroke patients provided by a team of physicians, residents, nurses and ancillary personnel.
Experience/Education Requirements:
- Current license as Registered Nurse in the State of Illinois required.
- Current CPR and ACLS certification required.
- Certification in emergency nursing preferred.
- Current experience in caring for neurology patients.
- Current National Institute of Health Stroke Scale(NIHSS) Certification required.
- After 6 months, knowledge in the care of stroke patients using the American Stroke Association Guidelines is required
