About the Role:
We are seeking a skilled Home Health Registered Nurse to join our team on a 13-week contract. The ideal candidate will have experience providing home-based care to patients, managing a caseload of 20-25 cases per week, and completing all documentation on the same day.
Responsibilities:
* Provide high-quality patient care in a home setting
* Develop and implement individualized care plans
* Conduct regular assessments and evaluations
* Collaborate with healthcare teams to ensure seamless care transitions
Requirements:
* Valid PA RN License
* 2 years of similar experience
* OASIS experience
* Familiarity with Oasis E post-Jan 2023
* No more than a 1-year employment gap
* Experience with Homecare Homebase
* BLS certification
