Position: Certified Home Health Aide (CHHA)
Locations: Middlesex, Union, Somerset County
Shifts:
Full Time
Monday- Friday:
7am-3:30pm
PT or Per Diem :
Monday - Friday 3:00-7:00PM or 4:00 - 8:00PM
Saturday- Sunday 7am-3:30pm, 3:00-7:00PM or 4:00 - 8:00PM
Starting Wage: Based on experience
What You'll Do:
As a Certified Home Health Aide at Grace Healthcare Services, every day is an opportunity to make a difference. You'll provide:
- Personal Care: Assist patients with bathing, grooming, dressing, skin care, and mobility support.
- Meal Preparation: Create nutritious meals tailored to individual dietary needs.
- Home Management: Maintain clean, safe, and comfortable patient living spaces.
- Documentation: Use Celltrak to accurately record patient care activities and ensure smooth communication.
- Team Collaboration: Work closely with RN Case Managers to monitor and report changes in patient conditions.
What You Need:
- NJ Certified Home Health Aide (CHHA) License: Must be in good standing.
- Communication Skills: Ability to read, write, and fluently communicate in English.
- Physical Readiness: Capable of lifting, pushing, and pulling up to 50 lbs.
- Flexibility: Willingness to travel within Ocean County, NJ, to bring care to those who need it.
