Position : Certified Home Health Aide (CHHA)
Locations : Middlesex, Union, Somerset County
Shifts : Full Time
Monday- Friday : 7am-3 : 30pm
PT or Per Diem :
Monday - Friday 3 : 00-7 : 00PM or 4 : 00 - 8 : 00PM
Saturday- Sunday 7am-3 : 30pm, 3 : 00-7 : 00PM or 4 : 00 - 8 : 00PM
Starting Wage : Based on experience
What You'll Do :
As a Certified Home Health Aide at Grace Healthcare Services, every day is an opportunity to make a difference. You'll provide :
- Personal Care : Assist patients with bathing, grooming, dressing, skin care, and mobility support.
- Meal Preparation : Create nutritious meals tailored to individual dietary needs.
- Home Management : Maintain clean, safe, and comfortable patient living spaces.
- Documentation : Use Celltrak to accurately record patient care activities and ensure smooth communication.
- Team Collaboration : Work closely with RN Case Managers to monitor and report changes in patient conditions.
