Job Summary
The Outreach Program Coordinator represents the welcoming spirit of Grace’s Table to community partners and mamas. The position includes four primary responsibilities: (1) oversee outreach efforts including cultivating community connections/partnerships; (2) intake/welcome of new mamas and inquiries; (3) facilitating on-site outreach activities; (4) function as a point of contact and/or Family Advocate for mamas within the program.
Responsibilities
- Develop and maintain relationships with local human services, education, and career development organizations.
- Represent Grace’s Table at community/partner events, as needed.
- Manage inquiries/referrals of new mamas.
- Ensure on-going communication with new/transient program participants.
- Provide leadership and guidance for GT staff and volunteers serving during Outreach program activities.
- Collaborate with Grace’s Table leadership to develop/implement/monitor program outcomes.
- Develop community resource and goal attainment plans with mamas on your case load.
- Complete routine charting in case management system related to mamas on your case load and/or attend groups.
- Coordinate and facilitate weekly groups.
- Collaborate with Grace’s Table team to identify areas of concern/opportunity and solutions.
- Assist in the creation of and the execution of special events.
- Maintain mama-focused communication channels related to Outreach programming.

Qualifications
- Bachelor’s degree or equivalent experience
- Two years of case management or program facilitation is preferred
- Bi-lingual/Spanish-speaking preferred
- Experience working with at-risk populations is preferred
- Must pass rigorous background checks
- Valid driver’s license and current automobile insurance
Physical Demands
- Physical requirements include a typical office environment including sitting at a computer screen and exerting up to 30 pounds of force occasionally.
- Must be able to access and navigate each department at the organization's facilities.
- Ability to climb, balance, stoop, kneels, crouch, and reach.
- The job entails standing for periods of time.
