What are the responsibilities and job description for the PER DIEM HOME CARE RN position at Gracious Hearts Home Care?
This is a Per Diem Role (As Needed). RN will be doing Client assessments,reassessments, any necessary paperwork and training/orientating new caregivers . RN will be able to make own schedule but will need to be able to schedule new client assessments within 48 hours of receiving new referral.

Flat Pay Rate
$85 Assessments
$65 reassessments
orientation/caregiving training (negotiable per hour rate) (classes will be twice a month and no longer then 3 hours)

Job Summary:
We are seeking a compassionate and dedicated Home Care Nurse to join our team. As a Home Care Nurse, you will provide essential medical care and support to patients in the comfort of their own homes. Your role will involve assessing patient needs, administering medications, and coordinating with other healthcare professionals to ensure the highest level of care.
Duties:
- Complete new client assessments, care plans, and updates as necessary.
- Complete new hire training, orientation and skills checks.
- Completes documentation timely, accurately, and at the point of care, according to industry standards, conventions and guidelines.
- Schedule, document, and complete field staff supervision visits quarterly on all caregivers.
- Ensure caregivers are following proper procedures, care plan and agency regulations. Provides training as needed.
- Provides assistance and direction to problems encountered by both office staff and field staff.
- Assess the needs of clients and the abilities of field staff to ensure the best possible care for clients.
- Coordinate case management of clients in collaboration with Agency Director and Office staff.
- Follows company policies and practices as outlined in the Employee Handbook and/or applicable employment agreement.
- Performs additional related duties as assigned.
- Educate clients and their families on managing chronic conditions and promoting overall wellness
- Coordinate with other healthcare professionals to ensure continuity of care
- Provide emotional support and comfort to clients and their families
- Qualifications: - Valid nursing license in the state of NC. SC license is a - Experience in non-medical home care is preferred or a related field - Proficiency with technology and software- Excellent communication and interpersonal skills - Ability to work independently and prioritize tasks effectively - Strong critical thinking and problem-solving abilities - Compassionate nature and a genuine desire to help othersNote: All positions at our organization are paid positions.
