Job Summary:
This role reports to clinic nursing management and is responsible for ensuring competent, compassionate, and individualized nursing care for patients with substance use disorder (SUD), with some focus on addressing neonatal abstinence syndrome (NAS). This role collaborates with all members of the care team to ensure comprehensive and coordinated patient care and seamless care transitions. The SUD Care Coordinator RN will be an ongoing resource between patients and providers for pain care and medication assisted therapy (MAT) management. This position is a part of the Health Resource Services Administration (HRSA) Rural Communities Opioid Response Program (RCORP) grant where funding will be used to strengthen and expand SUD/opioid use disorder (OUD) prevention, treatment, and recovery service delivery in our community.
Grand Itasca's RCORP project will:
(1) Reduce structural and systems-level barriers within the rural target service area to
(a) Increase access to behavioral health care, specifically SUD/OUD, services for rural pregnant and postpartum persons and their families; and
(b) Address community risk factors and social determinants of health.
(2) Strengthen the quality and sustainability of behavioral health care services for rural pregnant and postpartum persons and their families in the service area by implementing coordinated, evidence-based, family-centered SUD/OUD and other services.
JOB DUTIES:
1.Follows the RN process for nursing assessment, diagnosis, planning, implementation of care, and evaluation of patient progress to function in a full scope RN role to support providers in the clinic.
2.Works collaboratively with providers to triage patients, identify patients' level of care needs, provide care coordination, and required interventions using assessment, critical reasoning, and clinical judgement.
3.Assess and medically manage intoxication and withdrawal from substances using standardized screening and assessment tools/scales.
4.Manages, understands, and reviews with patients opiate and opioid medication contracts for patients with SUD.
5.Provide health education to facilitate patient treatment, including but not limited to medications for SUD and PrEP (Pre-exposure prophylaxis), safer drug use, and prevention of infections.
6.Provide and administer outpatient pharmacotherapy for SUD, including opioid antagonist or agonist therapy such as naltrexone, buprenorphine, and methadone.
7.Work with the pharmacy and the outpatient team to obtain medication orders, prescriptions, refills, and communication as needed.
8.Consult with members of the treatment team and other treatment professionals to devise an individualized plan for optimal psychosocial treatment for individual patients.
9.Review patients' charts and follow-up with patients and members of the health care team as needed to ensure that patients are current with all necessary medications, tests, screenings, and follow-up care.
10.Track and follow-up on critical referrals and test results with patient and care team.
11.Document nursing assessment, interventions, plan, and outcome in the patient record in a timely manner.
12.Evaluate effectiveness of care and interventions for patients with SUD concerns in collaboration with the health care team.
13.Communicate with members of the health care team, including inpatient and outpatient teams, to ensure continuity in care transitions.
14.Knows the key staff available to help patients with financial and emotional support; arranges for patients to meet with these resources people, if needed.
15.Respect and support patient and family's cultural beliefs and values, individual needs, health goals, and treatment preferences to support patient's treatment.
16.Other duties in support of unit patient's plan of care and consistent with full scope RN practice in accordance with Minnesota Nurse Practice Act.
Qualifications
1.Has a current RN license to practice nursing in the state of Minnesota.
2.Demonstrated knowledge of the Minnesota Nurse Practice Act and the Scope of Ambulatory Care Nursing.
3.Demonstrates clinical competence in providing direct patient care and patient and family education as well as assisting the physician in the delivery of patient care.
4.Has current BLS certification (or is willing to obtain within two months of employment.)
5.Demonstrated ability to manage the complex needs of patients with chemical dependency.
6.Experience managing patient schedule preferred.
7.Experience supporting individuals with substance use disorder preferred.
8.Understanding of the needs of patients with substance use disorder and chemical dependency.
9.Understanding of medication assisted therapy (MAT) and ability to be a liaison between patients and providers on MAT.
10.Demonstrated competence in managing patients using Patient Care Protocols.
11.Ability to manage change in a positive patient centered manner.
12.Clinic experience preferred.
13.Demonstrated ability to follow and adhere to Grand Itasca Clinic and Hospital policies, vision and mission.
14.Maintains reliable and consistent attendance.
