About the Role:
- This role requires a strong understanding of medication-assisted therapy (MAT) and the ability to communicate effectively with patients and healthcare providers.
- You'll work closely with the care team to develop and implement individualized care plans, promoting patient-centered care and positive outcomes.
The RCORP project focuses on strengthening and expanding SUD/opioid use disorder prevention, treatment, and recovery service delivery in the community.
JOB DUTIES:
- Conduct thorough assessments of patients' physical and emotional needs.
- Develop and implement individualized care plans, incorporating evidence-based practices and patient preferences.
- Collaborate with healthcare providers to ensure seamless care transitions and coordinate follow-up appointments.
- Provide education on SUD, MAT, and other relevant topics to promote informed decision-making and adherence to treatment plans.
- Maintain accurate and up-to-date patient records, adhering to HIPAA guidelines and Grand Itasca's policies.
Requirements:
- Current RN licensure in Minnesota.
- Expertise in ambulatory care nursing and chemical dependency.
- Knowledge of Minnesota Nurse Practice Act and Scope of Ambulatory Care Nursing.
- BLS certification (or willingness to obtain within two months of employment).
