Current opening: Monday - Friday 3 p.m. - 11 p.m. Full Time. Evening shift differential.
The LNA reports to the Hospice House Clinical Manager, and delivers age appropriate quality of life care to residents in facility settings. The Hospice House Aide will effectively interact with residents, family members, facility staff and other healthcare team members.

Essential Functions
Follows care plan in providing basic care and services to the resident including assistance with Activities of Daily Living (ADL): walking, bathing, shaving, mouth care, grooming, eating transfers, and management of incontinence.
Communicates with nurses involved in the resident’s care plan; observes, records and reports changes in resident’s condition, status, or other pertinent information to the nursing staff.
Does laundry, housekeeping and maintenance to assure a safe, orderly, clean and comfortable environment.
Provides supervision to residents with cognitive deficit.
Ensures residents have the opportunity to engage in cultural, spiritual, physical, political, social and intelligent activities as the residents’ desire.
Maintains calm, rational manner when dealing with others, even in situations involving conflict or crisis.
Documents care provided to the resident accurately in accordance to the agency’s guidelines.
Works independently on individual assignments yet support the needs of the total work unit.
Complies with the Agency’s documentation, time entry scheduling requirements
Communicates clearly and courteously with others including client, family members, and other agency staff.
Practicing safe work habits.
Attending Agency meetings and in-service programs.
Requirements
Minimum Qualifications and Requirements
High school diploma/GED, accompanied by current New Hampshire LNA license
Ability to meet all Post-Offer Pre-employment requirements (including physical capacity exam, background checks, etc.)
If hired, be available Monday-Friday for the first weeks of employment to complete the required training.
Salary : $17 - $25
