Job Title : Rehabilitation Therapy Position
Job Specialty : Physical Therapy Assistant
Job Duration : 13 Weeks
Shift : Monday to Friday, Days
Guaranteed Hours : 32 Hours per Week
Experience : Skilled Nursing Facility (SNF) Experience Highly Preferred
License : State License Verification Required
Certifications : N / A
Must-Have : Rehab Optima Experience Preferred
Job Description :
- Ensure prescreen documentation is completed and attached for application consideration.
- Attach resume and license verification for submission.
- Provide three references with the application.
- Complete county criminal background check for the past seven years.
- Perform federal and multi-state criminal searches covering seven years or more.
- Conduct a nationwide sex offender search for the past seven years.
- Perform HHS / OIG and GSA searches.
- Complete social security number trace.
- Provide 2-step PPD, Quantiferon test, or proof of positive PPD with chest X-ray.
- Undergo SAM search.
- Washington State background check with fingerprints will be managed by the facility.
- Position urgency is normal.
- Bilingual and holiday coverage are not required.
