Position Summary:
The RN Care Navigation Manager will work with and directly oversee the Clinical Care Navigation team in a post-acute care/telephonic setting; focusing on transitions of care (TOC) for patients leaving the hospital setting who are at risk for readmission due to the complexity of their diagnoses and access to information and support to manage their health. The RN Care Navigation Manager will be responsible for working directly with patients to address care needs, educate patients on management of their conditions, review available medical records to ensure compliance and understanding, communicate as needed with their care team, and following up after clinical care. Additionally, the RN Care Navigation Manager will be responsible for the day-to-day supervision and support of the Clinical Care Navigation Team. This includes coaching, reviewing interactions, and updating the collected data and/or process for such as needed.
Duties and Responsibilities ( including but not limited to):
- Consult on clinical cases/ review with peers appropriate to licensure (LPN) and offer guidance on best practice and interventions
- Have a strong understanding of company/department goals and share updates with assigned team
- Refer to/advise on departmental workflows with staff as appropriate to confirm understanding and adhere to departmental time frames
- Assign/dispatches tasks to designated team members; ensuring follow through
- Offers first-line support to team members to include guidance, support and resources
- Act as workflow/ EMR subject matter expert and ensure adherence and understanding; also review among team for Clinical Care Navigation duties.
- Offer immediate clinical support in an emergent situation where emergency services are required for patient in real time
- Attend and participate in candidate interviews as needed
- Coach team members to provide feedback and tips to improve performance
- Promotes open dialogue and teamwork amongst members
- Creates a supportive workplace atmosphere
- Adheres to confidentiality expectations
- Ensures all employees receive proper training and facilitates planning during onboarding process as well as required maintenance training in collaboration with other leads
- Monitors team progress and elevates any questions/concerns/wins to Associate Director/Director
- Facilitates continuing education and opportunities related to licensures
- Manage schedules, PTO approvals, Annual Reviews
- Additional duties as identified by Associate Director
Qualifications:
- Valid and current New York State Licensed RN
- Bachelor’s degree in nursing preferred; Associate degree in nursing is minimum requirement.
- 3-5 years’ care management and/or managed care experience in one of the following settings: acute inpatient, post-acute settings, homecare, ambulatory care management, or managed health plan preferred.
- One (1-2) years of supervisory experience preferred
- Must have ability to travel to and from office for required training/ meetings
Required Skills/Abilities:
- Multitasking and prioritization working in a high-volume environment
- Understanding and implementation of HIPAA and Confidentiality requirements
- Excellent interpersonal and organizational skills
- Clear communicative skills as well as astute listening skills
- Compassionate and patient centric approach
- Proficiency in EMR system(s), Outlook and data entry experience preferred
- Basic PC skills (MS Word/Outlook/PPT/Excel)
- Knowledge of Federal and State regulations for Medicare and Medicaid and other national and state funded programs.

Pay range: $90,000 - $110,000 annually
