What are the responsibilities and job description for the Manager, Non Clinical Care Navigation position at Great Lakes Integrated Network, Inc.?
Position Summary:
The Manager, Non Clinical Care Navigation will work with and directly oversee the Non-Clinical and Surgical Care Navigation teams in a post-acute care/telephonic setting; focusing on transitions of care (TOC) for patients leaving the hospital setting who are at risk for readmission due to the complexity of their diagnoses and access to information and support to manage their health. Additionally, the Manager, Non Clinical Care Navigation will be responsible for the day-to-day supervision and support of the Non-Clinical and Surgical Care Navigation teams. This includes coaching, reviewing interactions, and updating the collected data and/or process for such as needed.
Duties and Responsibilities ( including but not limited to):
- Guide the Care Navigation team to reach goals; both established and potential and monitor their progress
- Process weekly reports to monitor staff productivity
- Evaluate reporting of patient data
- Chart auditing to ensure accurate, high-quality care
- Assist with ( or be responsible for) all assigned projects and programs related to Non Clinical Care Navigation team as it relates to staffing, coaching, interviewing, onboarding, orientation, time, and attendance.
- Act as a liaison between GLIN Care Navigation team and GLIN network partners including providing practices, hospitals, and skilled nursing facilities
- Work with Associate Director, Care Navigation Services, on corrective action issues as well as write and administer annual performance reviews
- Gain training to be named a subject matter expert on the current EMR platform and be the point of contact; intermediary between Care Navigation Team and vendor
- Deliver exemplary customer service in order to provide a positive patient experience across the organization
- Encourage open communication
- Develop department policy and procedure
- Manage schedules, PTO approvals, Annual Reviews
- Additional duties as identified by Associate Director
Qualifications:
- Experience (Minimum needed): 5-7 years in Healthcare Setting; 2-4 years’ experience in management in a healthcare setting preferred
- Education: Bachelor’s degree in business, management, or related field preferred
- One (1-2) years of supervisory experience preferred Must have ability to travel to and from office for required training/ meetings

- Multitasking and prioritization working in a high-volume environment
- Understanding and implementation of HIPAA and Confidentiality requirements
- Excellent interpersonal and organizational skills
- Clear communicative skills as well as astute listening skills
- Compassionate and patient centric approach
- Proficiency in EMR system(s), Outlook and data entry experience preferred.
- Basic PC skills (MS Word/Outlook/PPT/Excel).
- Knowledge of Federal and State regulations for Medicare and Medicaid and other national and state funded programs.
- Strong presentation skills
- Demonstrated analytical skills

Pay range: $90,000 - $100,000 annually
