Job Description
Summary:
The Program Manager Patient Navigation will serve as a key leader in driving patient engagement initiatives across WNY. This role is responsible for designing, implementing, and managing comprehensive strategies that ensure sustained patient involvement post-screening and improved adherence to provider follow-ups. The Manager will oversee a team of patient navigators, collaborate with interdisciplinary healthcare teams, and lead efforts to enhance patient experience metrics, all while ensuring compliance with healthcare regulations.
Education And Credentials
Responsibilities:
BS or BA in Healthcare Administration, public health or a related field required. Master's degree in Healthcare Administration, public health or a related field preferred.
Experience

Proven experience in patient engagement, healthcare management, or related areas required. Strong leadership and team management skills required. Excellent communication and interpersonal abilities required. Proficiency in data analysis and reporting tools required.
