Greater Family Health is a multi-site federally qualified health center providing comprehensive medical, dental and behavioral health services within the Chicagoland area. Your Goal:
The primary purpose of this position is to facilitate coordination of care and care management for all Greater Family Health third-party managed care patients.
How You'll Help Patients:
- Completes care management screenings and risk assessments for applicable third-party managed care patients.
- Coordinates visits for applicable third-party managed care patients with their PCP for annual wellness evaluations, chronic care management, transitions of care or other required programs.
- Works directly with patients/family members to develop and implement a comprehensive care plan for applicable third-party managed care patients, which addresses medical and behavioral health needs, medication management and social determinants of health.
- Provides continuous monthly patient monitoring to assess the efficacy of the care plan for patients in applicable care management programs.
- Provides transition of care coordination for hospitalized patients of Greater Family Health in applicable third-party managed care programs.
- Provides patient education regarding labs, procedures and disease processes and any other health education information deemed necessary.
- Reports to all scheduled work sites and shifts on time and fully prepared to engage in all job responsibilities; location of work sites and shifts will change at the discretion of the supervisor.
- Engage in all offered training, consultations and supervisions made available. Fully and professionally respond to and implement directives and advice offered by all supervisory and training personnel, as well as professionally offered feedback from coworkers.

Qualifications
What You'll Need:
- Licensed Registered Nurse (RN) or Social Worker (LSW or LCSW) required.
- Associate's degree in nursing (ASN) or master's degree in social work (MSW)
Salary : $63,954 - $80,000
